This diagram is the typical appearance of meconium ileus at operation.  The relatively small caliber distal ileum is impacted white inspissated meconium.  In the more proximal ileum there is dilatation, which can be quite severe with the sticky thick green black colored meconium.  The meconium is sticky because of high protein content (85% relative to 7%).
